The sample extensions.lua was causing pbx_lua to fail to load when parsing
'app.goto("default", "s", 1)' because in Lua 5.2, 'goto' is now a reserved
word. This patch adds guidance to extensions.lua.sample and changed
'app.goto("default", "s", 1)' to 'app.['goto']("default", "s", 1)'.

Let's try that again, this time removing trailing whitespace and not leading
whitespace. I can't believe no one noticed.

It was pretty sparsely documented.
This update fleshes out the pbx_lua module, to
add the switch statements to the extensions in the
extensions.lua file, as well as removing them when
the module is unloaded.
Many thanks to Matt Nicholson for his fine
contribution!
